Fees:

Fees are due at the time of service.  Patients are responsible for any late, missed or cancelled appointments.  Please provide a 24-hour notice to cancel an appointment in order to avoid being charged for a full session.

Insurances:

Some medical plans cover out of network benefits and may reimburse a portion of your fees.  I will provide you with a monthly statement with documentation necessary for reimbursement.
